What You’ll See in a Patient Profile
IQ Summary
A 7-day clinical snapshot powered by Circadia’s AI. It highlights meaningful changes such as medication adjustments, infection markers, pending labs, missed doses, and overall condition shifts. This helps staff identify subtle trends and intervene early, often before symptoms escalate.
Vitals Graphs
Visual trends of respiratory rate and heart rate captured by the Circadia device, alongside PCC-documented vitals. This allows you to compare bedside measurements with continuous monitoring for a clearer clinical picture.
Flags Tab
A historical record of SBAR reports and flagged events. Each entry includes acknowledgment status, so you can track which escalations have been addressed and which still require follow-up.
Visits Tab
Documentation of H&P visits conducted by Circadia Nurse Practitioners, including assessments and outcomes. This helps facilities align monitoring data with in-person evaluations.
Motion & Bed Exits
Historical activity trends that show mobility levels, restlessness, or wandering behaviors. These patterns help identify fall risk, guide toileting schedules, and support personalized care planning. Bed-exit notifications can be enabled directly from here for high-risk patients.
Progress Notes
All clinical notes in one place — both Circadia-generated and PCC charting. This eliminates the need to jump between systems and ensures no documentation is overlooked.
Left Panel Information
Displays patient identifiers (name, room, demographics), current consent status, diagnoses, and options to add the patient to your Watchlist for closer monitoring.
How to Find Patient Profiles
Use the search bar at the top of CIP and enter the patient’s name or room number.
Or go to the Clinical Tab and click on the patient’s name.
📌 Consent Restriction: If a patient or family has denied Circadia consent, the Patient Profile will not populate and will show: “Resident did not consent.”
