Understanding the High Risk Patients List
Each column within the Morning Report provides context that helps your team quickly understand the patient’s current status and what may require attention:
Patient Name & Room – Identifies exactly who the patient is and where they are located. This ensures staff can act quickly, especially during huddles or when assigning tasks.
Length of Stay (LoS) – Displays the number of days the patient has been admitted.
Active Diagnoses – Shows a snapshot of the patient’s primary medical conditions. This column gives immediate clinical context without needing to open a full chart. Seeing diagnoses alongside risk scores allows staff to connect data trends with known conditions (e.g., COPD patients with rising respiratory rates).
PSN Flag – Indicates whether Circadia has escalated concerns in the past 72 hours, along with the status of that review. This makes it clear which patients have already triggered alerts, helping staff track what has been addressed versus what still requires action.
30-Day Risk Score (30D Risk) – Predicts the likelihood of hospital readmission on a scale of 0–100. This score is powered by PCC data (including progress notes, labs, vitals, and medication history). High scores signal patients who need closer observation and possibly early intervention to prevent transfer.
Last Change of Condition (COC) Note – Offers a brief excerpt of the most recent change in condition documented in PCC. This could include symptoms such as new shortness of breath, changes in appetite, or a recent fall. Having a quick snippet available reduces chart digging and highlights changes that may align with Circadia’s alerts.
Last Vitals – Shows the latest vital signs captured in PCC, along with the exact time they were recorded. This provides real-time context for whether clinical staff have recently assessed the patient.
Action Column – Includes a View Insights link that takes you directly to the patient’s CIP profile. This shortcut allows clinicians to move from a high-level report to a deep-dive view without wasting time navigating multiple systems.
Change in Condition (COC) Summary
For patients who have had a documented change in condition, the Morning Report includes a dedicated COC Summary. This section expands beyond the Patient List and provides the context needed to decide next steps:
Summary – A concise overview of the patient’s current status. It highlights symptoms observed, compliance with clinical care, and any treatments in progress. This gives a holistic view of what’s happening with the patient right now.
Assessments – A detailed list of new orders, communications between care providers, and pending or completed labs. This section also includes medication changes that have been made in response to the condition change. It connects the dots between the observation of symptoms and the medical actions being taken.
Recommendations – Circadia provides suggested next steps or considerations for proactive patient management. These may include closer monitoring, ordering additional labs, escalating to a provider, or updating the care plan. By consolidating clinical insights into actionable items, this section empowers teams to intervene earlier and prevent further decline.
