Circadia gives facilities flexibility in how consents are collected and documented. You can either:
Upload a scanned paper form that was signed in person, or
Obtain the consent digitally within the CIP, or
Both methods are valid — the key is ensuring all signed consents are legible, securely stored, and easily accessible for compliance review.

My Facility
The My Facility (Consents & Devices) tab in the Circadia App lets your team obtain, upload, and track patients' Circadia Consent status in real time. This helps ensure compliance without needing to manage separate spreadsheets or paper logs.
📝 Option 1: Upload a Copy of the Consent Form
When to use: Use this option if the consent was signed on paper or completed outside of the Circadia App.
How it works:
Scan or photograph the signed Circadia Consent to Monitor form.
Select the My Facility tab within the CIP.
Locate the patient’s name in the list, or use the search bar to enter the patient's name.
Click the Upload icon
under the Action column.Choose your file (PDF, JPG, or PNG) and select Open.
Choose the date the patient or representative signed the consent.
Click Confirm on the right of the window
Once the upload is complete, the Consent Status will change to Approved and appear with a green indicator.
Maintain a copy of the signed consent form in the patient’s chart (e.g., within your facility’s EHR) for your records.
💻 Option 2: Obtain Consent Within the App
When to use: Use this workflow when the patient or their authorized representative is present and able to sign digitally
How it works:
Go to Consents & Devices

Locate the patient in the list, or use the search bar to type the patient’s name.
Tap

A window will appear to complete the Consent Form
Review the details for the Consent on the left.
Confirm the information on the right. Correct any discrepancies before proceeding.
Patient name
MRN or DOB
Translator used (if applicable)
Signature date
Printed name
Relationship to patient
Click E-Sign at the bottom right of the form.
A signature window will open, prompting the patient/representative to sign.
After signing, select Approve at the bottom left of the page.
The consent will automatically save and update the Consent Status as Approved on the facility dashboard.

The Consents section consolidates all patients' consent to monitor status in one place.
How to View Progress:
Each row shows the resident’s status under Consent Status:
🟢 Approved – Consent signed or uploaded
🟡 Pending – Consent not yet received
🔴 Denied – Patient / Representative refused monitoring
Use the filter
to view only Pending, Approved, and/or Denied patients.Use the refresh icon
to update the dashboard in real time.To export your consent list, click the Download icon
in the top-right corner of the screen.This will download your list as a .CSV file, which can be opened in Excel or Google Sheets.
The CSV includes patient names, room numbers, admission dates, and consent statuses.
