Step 1 — Before You Begin
Before submitting a Parts Replacement Form, confirm the following:
Check Device Lights – The lights may be toggled off. Press the small button on the back of the device once (do not press and hold) to verify.
Ensure Proper Placement – Devices must be linked to the correct bed/floor. Confirm the device location by checking the yellow sticker.
Secure Power Cords – Make sure new power cords are installed and secured with cable clips to prevent damage or theft.
Step 2 — Access the Parts Replacement Form

Step 3 — Complete the Form
1. Facility details – Facility name and state.
2. Replacement details – Count of replacement parts.
3. Reason for replacement – Include a detailed description, including any relevant room numbers.
Enter the time, date, and reason for replacement.
4. Your contact information – Name, role, and email address for order updates.
Step 4 — Next Steps
